Personal Information	Legal name exactly as commissioned, mailing address, and contact information.
Commission Information	Commission number and commission expiration date.
Email Address	Email for official correspondence.
Eligibility Statements	Certifications that you meet the requirements to become an online notary.
Sample Notarized Document	Upload or provide the sample notarized document created after setting up your digital certificate and seal.
Electronic Signature	Your signature certifying that the application is true and correct.
Filing Fee	Electronic payment.

Make sure you can answer "yes" to each item below:

You already hold an active New York notary commission.
Your legal name and commission details match your notary records.
You have the commission number and expiration date available.
You have set up your digital certificate and seal.
You have created your sample notarized document.
You can receive official correspondence at the email address entered in the application.
You can certify that you meet New York's online notary eligibility requirements.
You are ready to pay the filing fee electronically.
